Zoom Patient Appointment Information

Due to COVID-19 precautions, we are transitioning our clinic appointments to telehealth visits.  The health and well-being of our patients and staff is our #1 priority. We would still like to see you via “video conference”. We will be using Zoom for our “video conferencing”.

You have the right to refuse telehealth without affecting your right to future care. All existing confidentiality protections shall apply to the service being provided by telehealth. Sharing of any patient identifiable images or information from the telehealth visit to researchers or other individuals will not occur without your consent.

If patient agrees they need the following:

  • A good email address (we NEED to have this listed on demographic page)
  • A device with webcam and microphone
    • Cell phone, tablet, laptop/desktop
  • Download “Zoom Cloud Meetings” in your app store or web browser
    • Cell Phone:  “Zoom Cloud meetings”
      • Follow the prompts to allow video and sound
    • Laptop/Desktop:
      • https://zoom.us/download  Choose the 1st option “Zoom for Client Meetings”
      • Follow the prompts to allow video and sound
  • You do NOT need to create an account or user ID, just download the app

In preparation for your appointment:

  • Have any questions you have ready for your provider
  • You will receive an email invitation with a link for the appointment shortly before your appointment time
    • Click on the link and follow the prompts
  • Even though you have an appointment time, your appointment may not start at the exact time
  • Please be available for 30 minutes prior to your scheduled appointment time and at least 30 minutes after your scheduled appointment time
  • Your nurse/CMA will start your appointment like usual, and then the provider will attend

Notice to Patients:

If you are experiencing these symptoms (fever, cough, shortness of breath) AND have traveled to or returned from these countries recently (China, Italy, South Korea, Iran, Japan, Taiwan, Thailand, Singapore) or have been in contact with someone who has traveled to one of these countries, or have been in contact with someone known to have to have COVID-19, please follow the below instructions:

  1. Keep yourself separated from others as much as possible, including your family & animals.
  2. Wear a facemask when around others and cover your coughs & sneezes
  3. Clean your hands often and for 20 seconds each time (sing the “happy birthday” song to yourself twice, or just count)
  4. Avoid sharing household items (like drinking glasses, utensils, towels, bedding)
  5. Clean all ‘high-touch’ surfaces daily (doorknobs, counters, tabletops, toilets, phones, keyboards, bedside tables)
  6. To obtain testing information and to self-report:
    1. To self-report regarding travel or symptom concern to Lancaster County Health Department at 402-441-8000
    2. If you reside outside Lancaster Count in Nebraska, please review the attached listing of each Nebraska County Health Department.

If you are unsure of your symptoms or risk level, the Nebraska DHHS is best equipped to answer your questions. Please call them before going to a clinic, or an urgent care center or the emergency department, in order to lower the risk of exposure to yourself and others.

University of Nebraska Medical Center Mobile Screening Application for COVID-19

UNMC has developed a mobile screening application that will allow people concerned that they or a family member have COVID-19 to screen themselves. Based on the information the user provides, the screening app will issue a “low-risk”, “urgent-risk” or “emergency-risk” assessment and guide the user through possible steps based on that person’s individual needs. The app also allows the user to press a button and share the information with a health care provider, family member or employer, if he or she chooses.
