As of November 1, 2023, Nebraska Pulmonary Specialties (NPS) ended its partnership with TogetherClinic for Chronic Care Management services and support. There is inherent value to our patients with multiple chronic illnesses to provide this additional level of support, but we recognize that this support needs to come from our trusted providers and team. NPS is working to bring this service, Chronic Care Management, in-house. Once more details are determined, an NPS Team member will reach out to all affected with that information. Thank you!
